I’m Isaac Berlin, FNP-C, a gay Family Nurse Practitioner providing LGBTQIA+ affirming care for adults in California and Alaska. My private practice, Be Well Medical Group, offers secure telehealth statewide in both states, with in-person visits available in West Hollywood, CA. I specialize in gender-affirming hormone therapy (HRT), gender affirmation letters, PrEP and DoxyPEP for HIV/STI prevention, mental health care (CBT, MI, and psychiatric medication management for anxiety, depression, trauma, and insomnia), Suboxone treatment for opioid use disorder, and primary care including chronic disease management, labs, STI screening and treatment. I have worked in LGBTQ+ health since 2012 through HIV prevention trials and community-based programs, and have been an NP since 2017. My care is inclusive of transgender, nonbinary, gender-diverse, Two-Spirit, kink, and polyamorous individuals. I am completing a Primary Care Psychiatry Fellowship at UCI School of Medicine to further strengthen my mental health practice. I offer longer visits, collaborative care, and clear communication centered on each patient’s goals and autonomy. In California, I accept Medicare Part B, Aetna PPO, Cigna PPO, Optum Behavioral Health, and pending Carelon. In Alaska, I am in-network with Medicaid, Aetna Behavioral Health, Cigna PPO, and Moda Health effective 12/15/25. Superbills available for out-of-network PPOs and cash-pay options. My goal is to provide safe, accessible, affirming care — especially for those who have been dismissed or misunderstood by the healthcare system.
