Kristin Evans

I am Kristin Evans, FNP-C, a board-certified family nurse practitioner and the founding provider of Groundwork Health in Chattanooga, Tennessee. I am a cis lesbian woman and a member of the LGBTQIA+ community, and I built this practice because I believe everyone deserves a provider who knows their name, takes their time, and never asks them to edit themselves to be seen. I have been providing gender-affirming hormone therapy for several years in primary care under an informed consent model. No therapist letters. No gatekeeping. No referral requirements. I work with transgender, non-binary, and gender-diverse patients across the full spectrum of gender experience, including patients with nuanced or partial transition goals. I also offer PrEP prescribing and management, STI testing and treatment, and comprehensive primary care through a direct primary care membership model that removes insurance from the equation entirely. Groundwork Health serves adult patients 18 and older via telehealth anywhere in Tennessee, with in-person care in the greater Chattanooga area beginning in 2027. I am currently working toward WPATH certification. A limited number of reduced-fee memberships are available for patients with financial hardship.
