Kristin Evans

I'm Kristin Evans, FNP-C, a board certified Family Nurse Practitioner and Precision Nutrition Level 1 certified coach based in Chattanooga, Tennessee. I founded Groundwork Health, a telehealth first direct primary care practice built around whole person care: small patient panels, unlimited visits, and real access to your provider, with no insurance hassle in the way. Primary care with me means looking at the full picture, not just the chief complaint. I integrate nutrition coaching directly into care because what you eat and how you move are part of your health, not separate from it. Affirming care is central to how I practice. That includes gender affirming hormone therapy delivered under an informed consent model, with no gatekeeping and no judgment. I welcome patients of every background, identity, and health goal. I'm also an out provider myself, which shapes how I show up for patients and why I built this practice the way I did.
